Yes, I think "cristata" (and "monstrosa") is a forma... A cristata-mutant plant looks very bizarr, because it's FORMA is special! (And you can find cristata forms in nature also, not just in nurseries...) BUT, it could be (and usually it is!) a cv. also, if it's COLOR is also special... This one is both, a forma cristata because it's special forma, and a cultivar. because it's special color, (and because it's from a nursery... :)... But I don't know it's exact cv.-name... :(
