Tree Help??

    I have a tree in my backyard that has been in decline for a while. It has got worse this year to the point where I am thinking it will have to be cut down. First of all, does anyone know what kind of tree it is? My neibor has the exact same tree on the other side of the fence and it looks healthy. Any ideas as to what has cuased my tree to die? There are mushrooms growing on the bottom of my tree on the portion of the trunk that is totally dead. Does this signify a fungus that may have caused the problem?

    Any insight would be appreciated!!

    Leaves look like those of bay willow (Salix pentandra).

    I cant see the fungus well enough to ID them from the pics provided but that type of growth in that location of the trunk would make me nervous indeed.
