1 yr old plant with new flower spike and 3 flowers suddenly has yellowing leaves which are falling off. Arial roots withering. Could this be to misting the plant, or type of fertilizer used, or too much water? How can you tell? This is my first orchid ever.

How cold are you keeping the plant?
What is the humidity the plant is in?
When did you last repot the plant?

    Answer the above three questions.
Check your potting soil condition: is it mucky? is it loose bark?
If mucky then repot immediately and don't worry about the flowers.
If you have good bark the plant is in then you can water daily. It definitely sounds like you are not watering much at all with aerial roots withering! If you mist the plant do it in mid to late morning.
Keep the Phally warm from 65F at night to 90F in the day time is good.

Thank you for the ideas. I know it does need repotting - it has not been done. I imagine the "soil" to be very dry as I may have been giving too much water. The flowers and all but 2 leaves are left on the plant. I have always had it in the centre of a room facing west. I have new medium here, so will repot this eve.
    Many thanks,Claire
