Identification: Help me ID this plant/tree

    I bought this plant almost 7 years ago. I got it at Lowe's and the only thing that I know about it was that it was labeled an Exotic Angel Tree. I just found out that this is the vendor name. I have just recently repotted it and it is not doing too well.
    Does anyone have any ideas on the type and any suggestions on how to care for it?
