    Hello,
I think that this might be a slightly light starved Trichocereus sp. (pachanoi?). Can someone please confirm?
    Thank you!

You are correct that it is a Trichocereus - extremely etoliated. Your best bet is to match the spine pattern to a healthy specimen (erect central spine, no radial spine at the top of the aerole, wooly aerole, shallow 'line' between the ribs, 5 or so ribs, smooth epidermis).

The more I look at it the more I think it might be Myrtlillocactus but without material to compare.... I'm just looking at pictures on the web.

That is a real possibility - seems to be a better match to the aerole details - and the way the pups attach to the adult.
