Third Dying PJM Rhododendren

    Help. I planted my third PJM Rhody this past spring in Zone 6 of SE Pennsylvania. The plant had a beutiful bloom and was doing great for 2-months.
    Just today I noticed that the leaves are all curling up and some are about to fall off.
    It looks like my previous two which completely died at the end of the last 2-seasons.
    Is there anything I can do to save this one? The blooms are beutiful in the spring and I would love to salvage it. I have attached a phot of the existing condition.
    Any suggestions on what is wrong and what can be done is most appreciated.

    Liable to be root rot due to water molds.
