please ID this mushroom, a Chinese boy died from eating it and her mom is in critical condtion.

I really would like to know the name of this mushroom. In the future, I would like to warn people from eating it.

I'd guess it is a Cortinarius (possibly an Inocybe).

 

Without seeing the underside of the mushroom, any ID would be a wild guess.
