Need to identify this plant and find its problem?

    I have had this plant for years, and it used to grow so well that I cut it down. It was small when I cut it, and when it rerooted it grew quite a bit.

    However, for the past few years, it has remained the same height as you can see in the photo I have attached here.

    Also, as you can see, some of the leaves in sporadic areas have completely dried out!

    I Live in Toronto Canada, and it faces a west window (all windows face west in my home and always have) and receives indirect sunlight.

    This has never been a problem for it before, so I dont really understand what the problem is!

    Can someone help me identify this plant and help pinpoint its problem?

    How do I heal my plant from this? Do I let the water stand for a day before using it and hold off on fertilizers??

    Collect rainwater and flush out the accumulated chemicals or just remove all the old soil and
    use fresh potting mix.
