Need help to ID Disease

    I have a much loved potted maple that had some (multiple) problem I cannot identify.

    We had a very wet winter here in central Ca. so most of my trees leafed out late but this maple seemed to leaf out much later than the rest. The smaller branches on the bottom seem to not develop buds or very little. Now the ones that have develped are stunted and falling off dead. I seen to have some greenish looking mold on some of the leaves underside and some type of bug I don't recognize. I've attached some (not so great) photos. Any advise on how to try to treat the tree as not to stress it anymore, or any ideas on how I can have the problem ID'd. Any help would be great Thx!

    I can't help with the disease, although I've seen similar symptoms on rootbound plants that have gone too long before repotting. I'm pretty sure, though, that the insect is a larval ladybug--one of the good guys, who eat aphids.
