I have had this plant for a few years and it hasn't grown much and after i recently replanted it it looks somewhat unhealthy. There are quite a few other plants like this in my area, and all are mostly woody and tall and have multiple leaves that are green. They flower seasonally and when the leaves are cut they excrete a milky sap. thanks for the help.

It's Plumeria, also known as frangipani. I don't know which species, but possibly Plumeria alba, judging by the leaf.
