Can someone please tell me what kind of plant I have? Why it's turning black and dying?

I have that plant, but I don't know what it is. If the stem is turning black and mushy, it is probably has rot. There are healthy leaves and roots forming along the stem. Looks like you could cut it above the rot and let it dry for a few days and then replant it in fresh light well draining soil. Like all succulents it needs good drainage. A grittier soil would be better.
