My Lemon Tree is dieing?

    About the tree:
    It is a potted, grafted Ponderosa lemon. Its pot has a volume of 5 gallons. I keep it outside in the nice Florida heat. The tree is currently bearing 3 small lemons. I make sure that the tree has adequate nutrition in the soil.

    Now on to the problem.

    The leaves are slowly turning yellow and falling off from the bottom to the the top.

    Yesterday I found a blood red maggot-like worm near the surface of my tree's pot.

    Some of the tree's died out roots are visable on the surface of the pot.

    You should know that I keep the tree on top of an improvised platform. This platfrom has spaces for rain water to collect and this collected rainwater has algee growing in it. This may or may not be an issue but I think it is an important detail.

    So if you can help me find a solution I would be so grateful!!!!
