My apple tree is dying, please help

    Hello,

    I planted a beautiful apple tree, 6 years ago with 5 different kinds of apple grafts on one tree. A self pollinating tree. I had 3 applies last year and this year it bloomed very well and I was expecting about 25 to 30 applies. Flowers started converted to fruit. Then something started happening to tree, leaves have these spots, remaining flowers are dying, even the small fruit seem to be dying. I am not sure what is a happening. \

    Please see the apple tree and leaves pictures attached. Please kindly help and let me know what I have to do to get the apple tree back.
