We are writing from a veterinary hospital, a patient has ingested this mushroom and came in with neurological signs. Bloodwork came back with liver and kidney damage. We are wondering if you could identify this mushroom for us.
In what type of environment was this mushroom located? Also, can you obtain a spore print? It looks very similar to a mushroom that was coming up in the grass of my backyard a couple of weeks ago, although the color is not quite as bluish as I remember. I tentatively identified my mushroom as Lepista (Clitocybe) tarda, which is supposed to be an edible mushroom that has a light tan or buff spore print. However, I'm not an expert in identification; I hope someone more experienced provides an opinion.
weird mushroom. I've posted the pictures to my list of experts. I am very interested in the case. Was the dog a puppy? They seem more susceptible.

Hi folks, Paul tells me it looks like Lepista tarda, not known to be toxic, so the area should be examined for other species that may have been ingested, and specimens examined microscopically. cheers, frog
