Can someone help us with our dying maple tree?

For the past 3 years, the tree seems to be dying limb by limb.

    Thanks

Might be an infestation of carpenterworm (Prionoxystus robiniae) or sugar maple borer (Glycobius speciosus).

Take a look at this picture, and site.

    http://www.barkbeetles.org
Yours looks rather like Arhopalus rusticus. Site does not provide photos of all listed species, but it's a start.

Treatment will depend upon ID of pest.

    Also try the excellent:
    http://bugguide.net
Click on the beetle image on the left side of screen.
You can submit your photo for ID, too.

Advise you to consult an arborist, who can identify the cause and severity of the problem, and advise treatment.
