Help identifying aggressive plant

    Hello, I'm a newbie here - I would be very grateful if someone could help me identify this very aggressive plant that is trying to take over my garden - and now my yard. It generally grows 3-4 inches high, with the flower stalks being maybe 6-8 inches high. It has yellow flowers right now (in southwestern Ontario), roughly the size of a nickel. The plant sends out roots/nodes very quickly.

    Every year I dig up as much as I can but it comes back the next year. Thank you in advance!
