Help for a sad tree

    Can you please help identify this tree? It was planted 3 years ago, and this is the first time it's looked so bad. The leaves are barely growing.

    The last picture is a closeup from a tree across the street-- I think it's the same type though I'm not completely sure.

    I don't know why the first tree looks so bad because it's been mulched, and it gets the same amount of water as other trees in the area.

    Looks like Quercus phellos - Willow Oak.

    The deformity of the leaves combined with the lush grass beneath makes me suspect lawn herbicides are the culprit.

    The first tree is located near the bottom of a hill. We added fertilizer to the lawn (which may have had weed control).

    Do you think it would help to water the tree a lot or get some tree fertilizer?

    I think it is pretty much a loss and should be replaced by a new specimen.
