Can someone help me identify what kind and name of my plant? It's also turning yellow on top, is it dying or might have a rotten root? Please help save my plant. Thanks.

Does that pot have a drain hole? If not, the culprit is likely that the roots are sitting in water. I'd recommend getting a proper pot with a drainage hole (and a plastic saucer so you don't ruin your floors) and transplanting it.
