I need help to identify and save this palm.
Been great for about 1 year. Indoors in California.
I added Palm, Ixora and Ornamental Plant Food 8-4-8 Plus Minors and this has happened to it. It is quickly fading.
Any ideas what kind of plant - what I can do to save it.
    Thank you UBC!

Looks like some sort of Date palm, to me.
Have you not been watering it??

Phoenix roebelenii Pygmy Date Palm. Check for spider mites, and how much light is it getting? Looks dark in the picture.

It needs water and light. If you're in LA, you should be OK to put it outside. The fetiliser may not have bene the best idea with it already in rough condition, but it may well help. Most important thing, though, is that it needs water light to draw up those nutrients in the first place.

I have never been able to keep these looking good as indoor houseplants, parts of it would look dry like yours, even with weekly waterings and good light.

Was this an indoor plant food you used? Maybe too much plant food shocked it? It looks dry could be like a lawn that got fertilized and then didnt get watered???

    Just ideas for your consideration
