Asking for help identifying a succulent...

    Hello all! My cousin has an adorable succulent. I'd always thought it had a name like "Baby Toes" or "Baby Tears", but in searching online I've found that those plants are not the one I'm looking for. This one is a pale green, sort of gray-green color. The "stems" are ultra thin and have minute little succulent "leaves", sort of flat, round shapes. When you barely touch the plant these "leaves" easily fall off. Any of the broken off stems can be place in water and root immediately. From time to time it blooms, sending out ultra thin flower stems with simple white flowers that aren't at all ornate...just somewhat star-like. Can you give me any info on this? I'd love to find one for myself. Thanks in advance.
