Pachypodium lamerei drying out?

    I have a pachypodium lamerei (Madagascar Palm) that I got 10+ years ago when it was 5 inches tall. It is now about 3 feet tall. The bottom foot's thorns look dried out but the top 2 feet's thorns are green/gray. The top 2 feet of trunk looks gray/green but the bottom foot has a large brown area (a few inches across). The very tip top has a hole about a half inch across by about 1 inch deep. The plant is not mushy anywhere.

    Should I be concerned about the dried-out thorns or the hole in the top?
